General comments
Vancomycine spiegel a 3-7 dagen (streefspiegel >15 ug/ml).
Overweeg toevoegen nystatine oraal 4 dd 500.000 IE (=5 ml) bij antibiotische behandeling > 1 week.
Antibiotica aanpassen op geleide van kweek.
Behandelduur bij coagulase negatieve Staphylococcus species (CNS-en), Streptococcus species: 2 weken.
Behandelduur bij S. aureus, gram negatieve staven en Enterococcus species: 3 weken.
