Comments:

Bij meerdere giften: spiegel 12 uur na 1e gift, vervolgdoseringen o.g.v. spiegel. 

Bij obese patiënten doseren o.b.v. adjusted body weight (voor Spaarne Gasthuis via knop in Epic). 
Voor RKZ:
100-120 kg: 480 mg
120-140 kg: 560 mg 
140-160 kg: 600 mg
160-180 kg: 680 mg
180-200 kg: 760 mg
200-220 kg: 800 mg

Bij ernstige sepsis op IC kan 6-7 mg/kg van toepassing zijn. 

Bij endocarditis 3 mg/kg doseren. 

Bij neonaten 5 mg/kg doseren (wijkt af van kinderformularium o.b.v. lokale research, Spaarne Gasthuis, Peeters et al., 2025 ).

Renal function-replacement therapy

CAPD: 1e gift standaarddosis, vervolgens interval verlengen op geleide van spiegel.

Hemo dialysis: 1e gift standaarddosis, vervolgens interval verlengen op geleide van spiegel. Op dialysedagen: bij toedienen voor dialyse normale dosis; bij toedienen na dialyse de helft tot een derde van de dosis.

CAV / VVHD: 1e dosis standaarddosis, vervolgens interval verlengen op geleide van de spiegel.
