General comments
Dosering vancomycine: zie Middelen- vancomycine en bij TDM vancomycine.
Postoperatief, posttraumatisch en endogeen: OOK altijd lokale therapie:
vancomycine 1 mg intravitreaal
+ ceftazidim 2,25 mg intravitreaal
+ vancomycine 25 mg subconjunctivaal
+ ceftazidim 100 mg subconjunctivaal
+ vancomycine 5% a 1-3 uur lokaal
+ ceftazidim 5% a 1-3 uur lokaal
Bij oogdruppels evt als alternatief voor ceftazidim 5% (niet commercieel verkrijgbaar): tobramycine 0,3%
