General comments

ernstige CAP: AMBU-65 3-5; PSI 5

Verricht binnen 12-24 uur een Pneumokokken- en Legionella urine antigeentest. Indien positief, behandelen conform respectievelijk community acquired pneumonie - Pneumokok of - Legionella. 

Indien er recente (<1 jaar geleden) kolonisatie is met ESBL: empirisch beleid daarop aanpassen.

Corticosteroïden
Corticosteroïden zijn geïndiceerd bij immuuncompetente patiënten met een ernstige pneumonie en bij één van de volgende criteria: 

  • Bij beademing of non-invasieve beademing (NIV); 
  • HFNO (Optiflow) met FiO2 > 50% 
  • Nonrebreathing masker met PaO2:FiO2 ratio < 300 (komt neer op 10-15L/min O2); 
  • PSI > 130 of AMBU-65 4-5

Gekozen kan worden tussen: 
prednisolon 1 dd 50 mg i.v. of p.o. OF hydrocortison 200 mg/24h continue infusie of 50 mg 4 dd iv OF dexamethason 4 mg 1 dd i.v. gedurende 5 dagen. 
Niet bij influenza of tuberculose.
